Right to a Paper Copy: You have the right to obtain a paper copy of this Privacy Policy upon request.

Right to File a Complaint: If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be retaliated against for filing a complaint.

Texas-Specific Privacy Protections

Under the Texas Medical Records Privacy Act (Texas Health & Safety Code Chapter 181), covered entities are prohibited from using or disclosing your PHI for marketing purposes without your prior written authorization. Texas law also prohibits the sale of your PHI and requires electronic disclosure of PHI to be encrypted.

Under the Texas Identity Theft Enforcement and Protection Act, we implement reasonable procedures to protect your sensitive personal information from unauthorized access, use, or disclosure.

Under the Texas Data Privacy and Security Act (effective July 1, 2024), Texas residents have additional rights regarding personal data collected through our website, including the right to know what data is collected, the right to delete personal data, and the right to opt out of the sale of personal data.
